Going home after hospital? We'll help you settle back in safely.

Our Transitional Care Community (TCC) Program is designed for older people who are well enough to leave hospital, but not quite ready to manage everything on their own.

The goal is to help you recover safely and confidently at home, with the right mix of professional care and support, for up to 12 weeks.

From the moment your care begins, we’ll work closely with you and your family to create a plan that suits your goals and daily needs. With the right support in place, you’ll be on your way to feeling stronger, more independent and back to yourself in no time.

What's included in the program?

  • Help with personal care like showering or getting dressed
  • Nursing support and medication help

  • Physiotherapy, occupational therapy or speech therapy

  • Domestic assistance such as cleaning

  • Emotional support and care planning from social workers

  • Coordination of any other services you may need

Who is eligible?

You may be eligible for the Transitional Care Community Program if you:

  • Are 65 years or older

  • Are currently a patient in hospital

  • Have been told you’re ready to go home

  • Would benefit from extra support for a short period of time

To join the program, you’ll need an assessment from an Aged Care Assessment Team (ACAT). Your hospital social worker can help with this process and refer you directly to Brightwater.

Once approved, you’ll be supported by a coordinator who will help ensure you have the right care, services and funding in place.

Who will be involved?

Your care team may include:

  • Registered and Enrolled Nurses

  • Care workers

  • Social workers
  • Physiotherapists

  • Occupational therapists

  • Speech pathologists

  • Therapy assistants

How much does it cost?

This program is funded by the government, but you’ll be asked to contribute a small daily fee.

For care provided at home, the maximum fee is $13.14 per day (equal to 17.5% of the single age pension).

If you're unable to pay, don’t worry — fee reductions are available and no one is turned away due to financial hardship. We’ll talk through everything before you begin so you know exactly what to expect.
