About Support Coordination

Brightwater offers support coordination and specialist support coordination. Whether it is helping clients access diverse therapy needs, overcoming critical situations that relate to accommodation, or family supports that need immediate attention, Brightwater’s team is specialised to solve complex problems. Once a client’s plan has been approved, our experienced support coordination team will help them build their independence.

Brightwater helps clients to:

  • Connect with both informal and formal supports.
  • Develop more independence to choose the right services for clients and get them started.
  • Explore activities, services and social connections that are important to clients.
  • Coordinate client support, including reviews if you have complex or changing needs.
  • Ensure the services you choose will work together to meet your goals.
  • Ensure you get the most out of your NDIS plan.

What we do best

Brightwater’s Capacity Building Team provides support to people with complex disabilities so they can live as safely and independently as possible while working towards individual goals. They provide services to people who have an acquired or neurological disability, physical disability or psychosocial disability, including:

  • Acquired brain injury (ABI)
  • Stroke
  • Younger Onset Dementia (YOD)
  • Huntington’s disease
  • Parkinson’s disease
  • Multiple sclerosis
  • Damage to the brain caused by substance misuse or disease
  • Mental health conditions and behaviours
  • Physical disability.
